It is important to understand how your health insurance coverage works and that you provide us with current health insurance and personal information. Our medical team will verify your insurance eligibility to protect you from claims that might not otherwise be paid. Please inform us immediately of any change in your insurance plan or personal data. If you are unable to complete the pre-visit forms at home, please allow enough time prior to your doctor’s appointment to complete the paperwork in the office.

Whether you are new to Norwalk Medical Group or a returning patient, it would be helpful to take the following steps to ensure your are well prepared for your doctor’s visit and to help us be sure we address all your concerns.
In an effort to get the most from your doctor’s appointment, we suggest you know and familiarize yourself with:
Please have your current insurance card with you at your doctor’s visit. If you are not paying through insurance, please bring a check, cash or credit card so you can pay in full at the time of service.
At the time of your initial office appointment, you are required to sign the following forms:
Please take the time to fill out the forms that apply to you, if you have the time, and bring them with you at the time of your visit.
Please bring an updated list of all your medications, including the name of the prescription, the dosage, how often you are taking it, and the prescribing physician.
When you make your doctor’s appointment, please provide a specific reason for your visit. This will ensure we have scheduled adequate time for your appointment. It may be helpful to bring a list of issues and concerns that you would like to discuss with your doctor.
If you have a health condition that requires home monitoring, such as diabetes or high blood pressure, please bring a copy of daily recordings to your visit to review with the doctor.
If you are coming for a follow up doctor’s visit based on information or testing done by another physician or outside of our office, please be sure to bring or have your results faxed to our office. This excludes most testing done at Norwalk Hospital as we can access those records directly.
If you are a new patient or are transferring your records from another office, please bring a copy of your records with you to our office.
If you must cancel or change your doctor’s appointment, we ask that you notify us at least 24 hours prior to the appointment so other patients may be scheduled. Please be aware that there is a charge of $50 for cancelled or missed office visits. Complete physicals or consults cancelled or missed without 48 hours prior notice will be charged $75.
